Job summary

Presbyterian Homes & Services is seeking an On-Call Speech Language Pathologist at Carondolet Village in Saint Paul, MN. You will assess residents, develop treatment plans, and provide quality speech-language pathology services across settings including TCU/SNF, assisted living, and home health.

You’ll collaborate with an interdisciplinary team, document accurately in the EMR, and ensure smooth discharges. This role offers weekday hours, competitive pay, and opportunities for growth within PHS.

Qualifications

• Graduate from an accredited Speech Language Pathology program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.

Responsibilities

• Conduct a comprehensive assessment & evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to achieve highest quality of life.
• Conduct screens and provide education to residents/clients, families, caregivers, and staff.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate and timely documentation & billing in electronic medical record.
• Demonstrate understanding and knowledge of payer sources and regulatory requirements.
• Complete and document supervisory visits per regulations.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
