Job summary

Calvary Hospital is seeking a Hospice Social Worker to provide medical social services and counseling to patients and families in a palliative care setting. The role requires MSW and the ability to travel across multiple territories to ensure continuity of care.

The ideal candidate will have at least one year of hospice or home care experience, strong interpersonal and communication skills, and a valid driver's license.

Qualifications

• MSW from an accredited school of social work with one year related experience.
• New York LMSW or LCSW eligible preferred.
• Excellent interpersonal and communication skills; ability to travel to multiple territories for continuity of care.

Responsibilities

• Provide medical social services, psychosocial and bereavement counseling to hospice patients and families.
• Support continuity of care across Brooklyn, Queens, Nassau and Manhattan territories.
• Coordinate with care teams and ensure patient and family needs are addressed.

Requirements

The Hospice Social Worker provides medical social services, psychosocial and bereavement counseling to the hospice Patients and families.

Qualifications

• Minimum one (1) year bereavement counseling experience is preferred
• At least one year of Hospice or Home Care experience preferred
• Excellent interpersonal, and communication skills
• Position requires ability to commute to multiple territories to ensure continuity of care
• A valid driver's license is required
• A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
• Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions
