Job summary

Calvary Hospital in Brooklyn seeks an experienced registered nurse to support hospice patients and their families with compassionate, comprehensive care. The role covers planning, implementing, documenting and coordinating patient care across multiple territories in a palliative setting.

You will ensure continuity of care with strong nursing practice and a valid NYS license. Driving and a vehicle are required for patient visits where applicable.

Qualifications

• Current NYS Registered Nurse license.
• Two (2) years experience in nursing which includes medical-surgical, oncological, palliative and community home health/hospice.
• At least one (1) year of community hospice or home health experience is preferred.
• Position requires ability to commute to multiple territories to ensure continuity of care; valid driver's license and vehicle required (except Manhattan territory).

Responsibilities

• Assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.
• Provides care coordination to ensure continuity across territories and settings.

Qualifications

• Hospice experience preferred.
• Position requires ability to commute to multiple territories to ensure continuity of care.
• A valid driver’s license is required. (This requirement does not apply to positions assigned exclusively to Manhattan territory)

Education

• Baccalaureate Degree in Nursing from an accredited school is preferred.

Salary:$113,576 - $140,076 annually
