Job summary

St. Croix Hospice is seeking a Registered Nurse to join our 24/7 care model as an on-call weekend nurse. This role involves after-hours visits, symptom management, and admissions on a 7 on/7 off overnight schedule with Fridays through Sundays shifts.

You will develop individualized care plans, administer medications, and educate patients and families while maintaining accurate documentation. A current Indiana RN license and CPR are required.

Qualifications

• Graduate of an accredited nursing program (associates, bachelor’s, or master’s).
• 1+ year of recent acute care experience; hospice/home care preferred.
• Current RN license in state of practice and CPR certification.

Responsibilities

• Respond to triage calls and provide after-hours nursing visits for symptom management.
• Complete evening/weekend admissions and update care plans as needed.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and educate patients and caregivers.
• Maintain timely clinical documentation of care, outcomes, and communication.
