Job summary

The Care Team Home Health & Hospice is seeking a Weekend On-Call Registered Nurse to serve Genesee County, MI. This role combines scheduled patient visits with on-call nursing duties, with remote work when not actively responding in the field.

The RN will assess patients, coordinate with the care team, educate families, and document thoroughly in the EMR, delivering compassionate end-of-life care in a supportive setting.

Qualifications

• Active RN license in the state of practice.
• Minimum of one year of nursing experience; hospice/home health preferred.
• Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field.
• Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred.
• Excellent communication and interpersonal skills with patients, families, and healthcare professionals.

Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs with timely nursing assessments and interventions.
• Perform on-call visits to assess changes in patient condition and adjust care plans.
• Coordinate care with physicians, caregivers, pharmacists, and the interdisciplinary team.
• Educate patients and families on medications, symptom management, and end-of-life care.
• Facilitate admissions, urgent situations, and end-of-life processes per hospice policies.
• Document patient assessments and care activities accurately in the EMR.
• Deliver compassionate, patient-centered care and support the needs of patients and families.

Schedule, Hours & Territory

• Dedicated Weekend On-Call RN Position
• Coverage Hours: Friday at 4:30 PM through Monday at 8:00 AM
• Combination of scheduled patient visits and on-call responsibilities
• Primarily remote when not actively responding to patient needs in the field

Qualifications: Key Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs by providing timely nursing assessments, interventions, and clinical support.
• Perform on-call visits to assess changes in patient condition, manage symptoms, and implement appropriate care plans.
• Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members to ensure continuity of care.
• Provide education and guidance to patients, families, and caregivers regarding medications, symptom management, and end‑of‑life care.
• Facilitate admissions, deaths, and other urgent clinical situations in accordance with hospice policies and regulatory requirements.
• Complete accurate and timely documentation of patient assessments, interventions, and care coordination activities in the EMR.
• Deliver compassionate, patient-centered care while supporting the physical, emotional, and psychosocial needs of patients and families.

Qualifications

• Active Registered Nurse (RN) license in the state of practice required.
• Minimum of one (1) year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred.
• Strong clinical assessment, symptom management, and critical thinking skills.
• Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field.
• Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred.
• Excellent communication and interpersonal skills with the ability to effectively support patients, families, caregivers, and healthcare professionals.
• Valid driver's license, reliable transportation, and current automobile insurance required.
