The Care Team Home Health & Hospice is seeking a Weekend On-Call Registered Nurse to support Genesee County, MI. The role combines scheduled patient visits with on-call responsibilities from Friday 4:30 PM to Monday 8:00 AM, with remote work when not actively in the field.

Qualified candidates are RNs with at least one year of nursing experience, strong clinical assessment skills, and familiarity with EMR such as Homecare Homebase. A compassionate, patient-centered approach is essential.

Qualifications

• Active RN license in state of practice.
• Minimum of 1 year nursing experience; hospice/home health experience preferred.
• Excellent communication and independent decision-making in field settings.

Qualifications: Key Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs by providing timely nursing assessments, interventions, and clinical support.
• Perform on-call visits to assess changes in patient condition, manage symptoms, and implement appropriate care plans.
• Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members to ensure continuity of care.
• Provide education and guidance to patients, families, and caregivers regarding medications, symptom management, and end‑of‑life care.
• Facilitate admissions, deaths, and other urgent clinical situations in accordance with hospice policies and regulatory requirements.
• Complete accurate and timely documentation of patient assessments, interventions, and care coordination activities in the EMR.
• Deliver compassionate, patient-centered care while supporting the physical, emotional, and psychosocial needs of patients and families.

Qualifications

• Active Registered Nurse (RN) license in the state of practice required.
• Minimum of one (1) year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred.
• Strong clinical assessment, symptom management, and critical thinking skills.
• Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field.
• Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred.
• Excellent communication and interpersonal skills with the ability to effectively support patients, families, caregivers, and healthcare professionals.
• Valid driver's license, reliable transportation, and current automobile insurance required.
