Responsabilidades

• Make emergency on-call visits to implement the nursing care plan and document per Medicare requirements.
• Participate in weekly team conferences with essential information and documentation.
• Support Hospice patients during non-office hours via phone and skilled visits as indicated.
• Maintain attendance, dependability and professional appearance with minimal supervision.
• Communicate effectively and demonstrate professional development.
• Handle equipment safely and respond properly in crises.
• Evaluate appropriateness for hospice care and admit patients if needed.
