Job summary

Great Nurses Home Care in Burbank, CA is seeking an experienced Registered Nurse Case Manager to provide essential home health nursing during non-business hours, including nights, weekends, and holidays. You will respond to urgent patient needs and maintain continuity of care across the Home Health experience.

The role suits a compassionate nurse who can work independently, assess symptoms, administer medications, and collaborate with the interdisciplinary team.

Qualifications

• Active RN license in California.
• Minimum 1 year Home Health or palliative care experience.
• Strong clinical assessment and autonomous decision-making.
• Empathy and caring communication with patients and families.
• Reliable transportation and valid driver’s license.
• Experience with EMR documentation.
• Bilingual in Spanish, Hmong, or other patient languages is a plus.

Responsibilities

• Respond to patient calls, triage needs, and deliver compassionate Home Health care.
• Assess and manage symptoms including pain, shortness of breath, agitation, and more.
• Provide skilled nursing care per plan of care and Home Health protocols.
• Administer medications and perform treatments as prescribed by the physician.
• Communicate and collaborate with the interdisciplinary Home Health team to ensure quality, coordinated care.
• Support families and caregivers through education, emotional support, and guidance.
• Accurately document all interactions, assessments, interventions, and changes in condition.
• Travel to patient residences to deliver in-person care when necessary.
• Participate in Home Health team meetings and ongoing staff development.
