Job summary

Elara Caring is seeking an After Hours/ On Call Registered Nurse to deliver compassionate hospice and home health care in patients' homes and communities. You will assess needs, coordinate with the Interdisciplinary Team, and provide high-quality care during after-hours shifts.

Required qualifications include an RN license, at least 1 year of clinical experience, and willingness to travel up to 50%. A valid driver’s license and reliable transportation are essential.

Qualifications

• Graduate of an accredited nursing program
• Current State RN license
• Minimum 1 year of clinical care experience
• Hospice or home health experience preferred
• Willingness to travel up to 50%
• Valid driver’s license and reliable transportation

Responsibilities

• Assess hospice and home health patients and families to identify needs and document findings
• Coordinate care with the Interdisciplinary Team and clinical supervisors
• Provide after-hours nursing care and support in patient homes
• Document care, changes in condition, and outcomes in clinical records
• Educate patients, families, and facilities on care plans and pain/symptom management
