Job summary

Continuum Hospice is seeking a Full Time On-Call Registered Nurse (RN) to join our team, delivering compassionate hospice care during critical moments in patients\' homes within San Jose and surrounding areas.

You will respond to diverse patient needs, coordinate care, conduct holistic assessments, document accurately, and support families through end-of-life care while maintaining compliance with regulatory standards.

Qualifications

• Must hold an active RN license in good standing.
• Bachelor's degree in nursing or equivalent; or one year of professional nursing experience within the last three years.
• Hospice/home health experience preferred but not required.

Responsibilities

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, with unwavering support.
• Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of physical, emotional, social, and spiritual needs.
• Maintain Accurate Documentation: Ensure thorough documentation in the home and agency records.
• Provide Support for Families: Guide and support families during the patient’s end-of-life journey.

Overview

Continuum Hospice is currently seeking a Full Time On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments. As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support.

Schedule: On Call for 7 Days on and 7 Days off from Friday at 5 pm - Monday 8 am, Monday 5 pm - Friday 8 am

Territory:San Jose and Surrounding Areas.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including a background in EMR.
• Possess and maintains current CPR certification.

Pay: Approx. $135k annually with pay differentials paid hourly; may vary with hoursworked.Thepayrange listedrepresentsa general guideline for the role and is not a guarantee of the final offer. Compensation will bedeterminedbased on the selected candidate’s relevant experience and the specific responsibilities of the position.
