Job summary

Continuum Hospice is currently hiring a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the Santa Cruz Area. This role involves in-home and facility visits, patient assessment, and collaboration with clinical leadership to ensure high-quality end-of-life care.

We offer a sign-on bonus and a comprehensive benefits package. Schedule includes Tue-Sat or Wed-Sun, 1pm-8:30pm, with on-call duties as needed.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from an accredited program; OR one year of professional nursing experience in the last three years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient (typing) and familiar with EMR systems.
• Current CPR certification and valid driver's license with reliable transportation.

Responsibilities

• Review clinical referral information and collaborate with Clinical Director on eligibility for hospice services.
• Provide pre-assessment and admission services; conduct home or facility visits as needed.
• Communicate with Intake, referring physicians, and Hospice Medical Director to ensure referral appropriateness and certifications.
• Deliver nursing care, emotional support, and patient/family education; encourage participation in the plan of care.
• Participate in On-Call duties as assigned by Clinical Director and Program Director.

Responsibilities

• Reviews clinical referral information and responds to request and inquires as appropriate. Collaborates with Clinical Director regarding the eligibility and appropriateness of client for hospice services.
• Provides information, pre-assessment, and admission services to patients who have been referred to the hospice; conducting pre-assessments visits in the home or healthcare facility as necessary.
• Communicates with the Intake Department, the referring physician and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
• Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient's plan of care to meet established goals.
• Participates in On-Call duties as assigned and defined by Clinical Director and Program Director.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
• One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$61.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
