Job summary

Ascend Health is seeking a compassionate Registered Nurse (RN) to join our Home Health & Hospice team in Idaho Falls and Pocatello. This on-call, full-time role focuses on after-hours triage, nursing visits, and family support in patients’ homes.

You will collaborate with the interdisciplinary team to ensure continuity of care during evenings, nights, and weekends, with a schedule of 7 days on/7 days off. Prior HH or hospice experience is preferred, with a Utah RN license required.

Qualifications

• Active RN license in Utah.
• Minimum 1 year nursing experience (Home Health or Hospice preferred).
• Ability to work independently and manage time.
• Reliable transportation and a valid driver’s license.

Location: Idaho Falls/Pocatello
Schedule: On-call 2 weeks per month; 7 days on/7 days off - Monday–Friday 5:00 PM–9:00 AM, and 24 hours on Saturday & Sunday

Position Overview

As an On-Call RN, you will provide after-hours triage, nursing visits, and support to patients and families in their homes. You'll collaborate closely with our interdisciplinary team to ensure continuity of care and timely response to patient needs during evenings, nights, and weekends.

Schedule

• On-call 2 weeks per month; 7 days on/7 days off

  • Monday–Friday: 5:00 PM – 9:00 AM

  • Saturday & Sunday: 24-hour coverage each day
