Job summary

WellLife Network seeks a Clinical Staff Member to provide psychiatric and medical care within the ACT team. You will assess, treat, and monitor clients, administer medications, and support treatment planning across behavioral health programs.

Responsibilities include conducting comprehensive assessments, delivering therapy, coordinating with physicians, and ensuring timely documentation in the EHR. On-call crisis coverage and community-based support are integral parts of the role.

Qualifications

• Bachelor's degree in nursing; RN licensure; at least 1 year of experience with individuals who have mental illness.
• Knowledge of ACT model and community-based behavioral health services.
• Valid New York State driver's license and access to a vehicle.

Responsibilities

• Conduct psychiatric, medical, and nursing assessments.
• Support prescribers with physical health assessments and referrals.
• Administer injections and medications; monitor reactions.
• Draw labs, record vital signs, and maintain MARs.
• Develop treatment plans with input from the client and team.
• Deliver individual, group, and family therapy and illness-management education.
• Coordinate with ACT specialists to integrate services.
• Document progress notes in the EMR within 48 hours.
• Provide crisis intervention and community liaison work.
• Attend daily team meetings and assist with case planning.
