Benedictine's Cerenity Senior Care-White Bear Lake campus is seeking a Physical Therapist for an on-call, casual role in White Bear Lake, MN. Position supports resident evaluations, ongoing assessments, and implementation of therapy interventions.

Responsibilities include developing care plans, discharge planning, and education for the health care team, residents, and families. Licensure must be active in the state and a Bachelor's degree in Physical Therapy is required to practice.

Competenze

• Current state licensure in Physical Therapy, in good standing.
• Bachelor's degree in Physical Therapy required.

Mansioni

• Develops and revises plan of care based on residents' needs and establishes functional goals, outcomes, and discharge plans.
• Plans for residents' discharge and anticipates long-term and/or short-term residents' needs.
• Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
