Job summary

West Gardena Post Acute is seeking an experienced Licensed Vocational Nurse (LVN) for an on-call role in Gardena, CA. You will deliver direct nursing care under RN supervision, administer medications, and monitor residents to ensure high-quality care.

Experience with med pass and Point Click Care is required. AM shift 7AM–3PM is currently available; we value teamwork, documentation accuracy, and compassionate patient support.

Qualifications

• California LVN license and current RN/ LVN licensure in good standing.
• Graduate of an accredited nursing program.
• Minimum 1 year experience in a skilled nursing facility, acute care, or inpatient rehabilitation.
• Strong clinical skills and ability to document care clearly.

Responsibilities

• Provide direct nursing care, including meds administration and vital signs.
• Develop and implement individualized care plans with the care team.
• Coordinate with physicians and therapists for patient care.
• Maintain accurate and up-to-date charting and documentation.
• Assist with activities of daily living (ADLs) as needed.
• Educate residents and families about health conditions and treatments.
• Respond to emergencies and provide first aid as required.
• Adhere to facility policies and regulatory standards.
