Job summary

West Hills Health and Rehab Center in Los Angeles, CA is seeking a Licensed Vocational Nurse (LVN) to join our clinical team. You will provide resident-centered care, supervise CNAs, and ensure accurate medication administration and charting.

The role supports a float schedule with shifts 7am-3pm and 3pm-11pm, requiring an active California LVN license and the ability to thrive in a fast-paced long-term care setting. PCC experience is preferred and growth opportunities are emphasized.

Qualifications

• Active California LVN license.
• Experience or ability to supervise certified nursing assistants.
• Ability to provide resident-centered care in a long-term care setting.

Responsibilities

• Provide leadership and supervision to CNAs.
• Oversee the care and wellbeing of residents.
• Prepare and administer medications as ordered by the physician.
• Complete charting and treatment plans/physician orders, etc.
• Admit, transfer, and discharge residents as required.
• Make rounds with physicians as requested/necessary.
• Orient and welcome new residents and families.
• Knowledge of Point Click Care (PCC).
• Other duties as assigned.
