Job summary

Ascend Hospice & Palliative Care KS in Overland Park, KS is seeking a dedicated RN On Call (Weeknights) to provide patient- and family-centered care in a home setting. Hours: Monday–Thursday, 4:00 PM – 2:30 AM.

You will observe and document patient needs, consult with the Hospice Nurse, and deliver care as prescribed, while maintaining communication with families and the IDC team. Required: active RN license in Kansas, 1 year clinical experience; LTC/Hospice preferred.

Qualifications

• Hold a valid and current RN license in the state of operation.
• 1 year of clinical experience.
• Long Term Care/Home Care/Hospice experience preferred.

Responsibilities

• Observes and documents the physical, mental and emotional needs of the patient and family on an ongoing basis.
• Consults with the Hospice Nurse regarding the patient's status and symptom management.
• Delivers physical and psychological care as prescribed by the attending physician and as approved by the IDC team.
• Monitors and documents patient/family input and response to care.
• Maintains continuity of patient care through ongoing communication with families and other health care professionals.
• Participates in Interdisciplinary Team Meetings and related meetings.
• Maintains sufficient home care supplies for patient care management.
• Collection of documentation and paperwork as requested by the Hospice Nurse.
• Assistance with clinical billing as deemed necessary.
• Participates in orientation of new hospice aides.
