Job summary

Ascend Hospice & Palliative Care KS is hiring an RN On Call Nurse for weekend days in Topeka, KS. The on-call nurse assesses, plans, implements, and evaluates patient care using the nursing process and coordinates with physicians and the hospice team.

Key duties include pain and symptom management, thorough documentation, medication administration, and collaboration with the Medical Director to determine patient eligibility and home care needs. Travel within the assigned territory is required.

Qualifications

• State Certified RN license required.
• Experience in hospice or home health care is REQUIRED.
• Must have excellent documentation skills and the ability to work independently.
• Strong communication skills.
• Valid driver\'s license and reliable transportation.
• Must be able to travel locally within assigned territory.

Responsibilities

• Assumes responsibility for patient visits or on-call schedule.
• Provides hospice nursing care focusing on pain control and symptom management.
• Initiates communication with attending physicians and hospice staff to coordinate care.
• Maintains up-to-date documentation at point of service.
