Pathways Healthcare is seeking an On Call Hospice Registered Nurse for weekend and night shifts in Kingston, PA. This part-time role offers base compensation plus visit rate and serves the Wilkes-Barre area with in-home patient care.

Responsibilities include administering medications, implementing care plans, triaging needs, and coordinating with physicians and the care team. Mileage reimbursement and 401(k) match are offered.

Qualificações

• 2+ years nursing experience (geriatrics preferred).
• New nurses welcome with training program.
• Licensed RN degree from an accredited program.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver’s license and insured vehicle.

Responsabilidades

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by physician.
• Implements and revises plan of care as patient status changes.
• Responds to after-hours calls during weekends 8:30 AM–8:00 PM.
• Assesses needs via triage and provides in-home care when needed.
• Collaborates with physicians and interdisciplinary team on the care plan.
• Communicates with patients and caregivers before, during and after treatment.

Job Description:

• Role: On Call Hospice Registered Nurse - Weekend/Nights
• Rate: Base Compensation + Visit Rate
• Main Office location: Kingston, PA
• Service Area:Wilkes Barre area.
• Schedule: Saturday and Sunday 8:00 pm – 8:30 am
• Job Type: Part-time

Qualifications:

• 2+ years’ nursing experience, preferably in geriatrics is desirable.
• New nurses, We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assess patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
