Job summary

Elara Caring is seeking a Registered Nurse for after-hours on-call in Michigan. The role focuses on delivering high-quality home health care with a 7-on, 7-off schedule and 1:1 patient care.

The RN will utilize OASIS experience and coordinate with a clinical team to ensure patient safety. Responsible for evaluating patients, communicating findings, and implementing care plans with a compassionate, mission-driven approach.

Qualifications

• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• Active RN license in the state.

Responsibilities

• Assess assigned home health care patients to identify physical, psychosocial, and environmental needs.
• Perform initial evaluation and re-evaluate nursing needs during each visit.
• Communicate findings to clinical supervisor, physician, and care team.
