Job summary

Cantex is seeking a PRN Registered Nurse for on-call hospice care in Livingston, TX. The RN will perform comprehensive in-home assessments and coordinate with the interdisciplinary group, patient, family, and referral sources to ensure quality end-of-life care.

Essential duties include developing and updating care plans, educating families, and monitoring patient needs. Strong nursing assessment, documentation, and English communication skills are required.

Qualifications

• RN license required or licensed per state.
• 2 years Hospice experience preferred.
• Strong communication and clinical documentation skills.
• Must be able to read, write, and speak English.

Responsibilities

• Conduct initial and ongoing assessments of patients’ needs.
• Coordinate and document care plans with the IDG and family.
• Educate patients and families about disease, end-of-life care, and nutrition.
