Job summary

Cantex is seeking an experienced Registered Nurse (RN) to provide skilled hospice care in patients' homes across Dallas. You will perform initial and ongoing nursing assessments, develop and manage plans of care, and coordinate with the IDT, physicians, patients, and families.

The role requires strong clinical assessment, independent work in the field, and excellent documentation and communication. Hospice regulations, case conferences, and quality care are central, with benefits including 401K,

Qualifications

• RN licensed per state requirements.
• Hospice experience preferred.
• HCHB experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Experience working within an interdisciplinary healthcare team.
• Knowledge of hospice regulations, documentation, and care planning preferred.
• Reliable transportation, valid driver's license, and current auto liability insurance.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients.
• Develop, implement, and update individualized hospice plans of care to ensure quality and continuity of care.
• Coordinate patient care with the Interdisciplinary Team (IDT), physicians, patients, and families.
• Educate patients and families on disease process, medications, symptom management, and end-of-life care.
• Determine scope and frequency of nursing services based on patient condition and care needs.
• Supervise and provide clinical direction to Hospice Aides and LVN/LPN staff.
• Maintain accurate and timely clinical documentation in the electronic medical record.
• Communicate changes in patient condition to physicians and interdisciplinary team members.
• Ensure compliance with Medicare hospice regulations, documentation standards, and agency policies.
• Participate in case conferences, IDT meetings, and care coordination activities.
• Monitor assigned cases to ensure compliance with third-party payer and hospice requirements.
• Promote hospice philosophy and provide compassionate, patient-centered end-of-life care.
