Job summary

Hospice of the Midwest in Edina/Monticello, MN is seeking an On-Call RN to provide compassionate, hands-on hospice care during after-hours, including weekends on a rotating, every-other Saturday and Sunday schedule from 8:00 AM to 8:00 PM.

You will collaborate with a Remote Triage On Call Team when available or deliver hands-on care independently, assess urgent symptoms, administer medications, and coordinate with physicians.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Must be a licensed driver with insured vehicle and CPR certification as required.

Responsibilities

• Provide 1:1 in-home or facility care to hospice patients during after-hours on-call.
• Collaborate with Remote Triage On Call Team if applicable, or deliver hands-on care independently.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols.
• Communicate with physicians about changes in condition and obtain orders as needed.
• Provide guidance and support to patients and families about current condition and next steps.
• Pronounce death when occurring during on-call hours if necessary and coordinate funeral home notification.
• Complete timely documentation and communication.
