Job summary

Trellis Supportive Care is seeking an experienced Registered Nurse – On-Call to deliver compassionate hospice care to patients and families in the Winston-Salem, NC area. You will assess patients, triage calls, and perform home visits as needed, while collaborating with an interdisciplinary team to ensure symptom control and dignified end-of-life support.

The role requires an active NC RN license, CPR certification, and strong clinical skills. Preferred BSN and homecare experience.

Qualifications

• Current North Carolina RN license.
• Current CPR certification.
• Bachelor’s Degree in Nursing preferred.
• Minimum of one year of clinical nursing experience preferred; homecare nursing experience strongly preferred.
• Strong assessment, critical thinking, problem-solving, organization, and communication skills.
• Valid driver’s license and required automobile liability insurance.

Responsibilities

• Provide nursing assessment, symptom management, treatment, education, and follow-up per plan of care.
• Triage calls and determine needs; arrange home visits as needed.
• Utilize clinical skills in wound care, lab draws, and PCA pumps.
• Identify changes in patient condition and coordinate with physicians and team for interventions.
• Provide emotional support and education to patients and families about disease progression and end-of-life care.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Collaborate with the interdisciplinary team to support quality hospice care.
