Job summary

Trellis Supportive Care in Winston-Salem, NC seeks an experienced Registered Nurse (RN) – On-Call for evening shifts to support hospice patients and families. As an On-Call RN, you will assess patient needs, triage calls, perform home visits when indicated, manage symptoms, and collaborate with the interdisciplinary team to ensure high-quality end-of-life care.

This full-time position requires NC RN licensure, CPR certification, and strong assessment and communication skills; homecare experience

Qualifications

• Current North Carolina RN license.
• Current CPR certification.
• Bachelor’s Degree in Nursing preferred.
• Minimum of one year of clinical nursing experience preferred; homecare experience strongly preferred.
• Strong assessment, critical thinking, problem-solving, organization, and communication skills.
• Understanding of hospice philosophy and patient/family needs.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Provide compassionate nursing assessment, symptom management, education, and follow-up per plan of care.
• Triage patient and family calls and make home visits based on clinical assessment.
• Utilize skills in wound care, lab draws, and PCA pumps.
• Identify changes in patient condition and collaborate with physicians and the interdisciplinary team.
• Provide emotional support and education to patients and families on disease progression and end-of-life care.
• Maintain accurate clinical documentation and ensure compliance with hospice regulations.
• Collaborate with the interdisciplinary team to address patient needs and support quality hospice care.
