Job summary

St. Croix Hospice in Iowa is seeking an experienced On-Call RN to join our end-of-life care team.

This salaried position covers evenings and weekends, delivering skilled nursing visits, admissions, and after-hours support to patients and families in home settings. You will assess patients, develop and revise Plans of Care with families, administer prescribed medications, provide education, and ensure comfort and dignity during transitions.

Qualifications

• Current RN license in state(s) of practice.
• Graduate of an accredited school of nursing.
• Ability to pass background, consumer report and drug test.

Responsibilities

• Provide after-hours hospice care visits and admissions.
• Develop and revise Plan of Care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family to support nursing and related needs.
• Maintain accurate clinical notes and ensure HIPAA compliance.
