Job summary

Legacy Hospice in Selmer, TN seeks a compassionate Registered Nurse to deliver high-quality end-of-life care to patients in their homes. You will support families with empathy, manage a hospice caseload, and participate in on-call rotations with the interdisciplinary team.

Requirements include an active RN license in the state, BSN or CHPN preferred, and 1+ year hospice/home health experience. CPR/BLS certification and reliable transportation are required.

Qualifications

• Active RN license in the state of employment.
• BSN preferred or CHPN certification preferred.
• Minimum 1 year nursing experience in hospice, home health, or related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification is required.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients and coordinate care with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management and end-of-life care.
• Monitor and update care plans based on ongoing assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians and care partners.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
