Job summary

Legacy Hospice, Inc. in Selmer, TN is seeking a compassionate Registered Nurse to deliver high-quality end-of-life care in patients’ homes while supporting families with empathy and respect.

You’ll manage a caseload with the interdisciplinary team, educate families, administer medications, and maintain meticulous documentation. This is a full-time, salaried role with competitive pay and mileage reimbursement.

Qualifications

• RN licensed in the state of employment.
• BSN or CHPN certification preferred.
• Minimum 1 year of nursing experience in hospice, home health, or related field.
• Strong interpersonal, clinical, and organizational skills.
• Active CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans.
• Manage a caseload of hospice patients coordinating with the IDT.
• Administer medications and treatments per physician orders.
• Educate patients and families on symptom management, medications, and end-of-life care.
• Monitor and update care plans based on ongoing assessments.
• Provide emotional and clinical support to patients and caregivers.
• Communicate with physicians, care partners, and community resources.
• Maintain accurate, timely clinical documentation.
• Participate in IDT meetings and on-call rotation.
• Supervise ancillary staff and ensure continuity of care.
