Job summary

Three Oaks Hospice is seeking a PRN On-Call Registered Nurse (RN) to provide compassionate in-home nursing care to patients across ages in their residences. You will coordinate with the interdisciplinary team, families, and referring agencies to ensure seamless care delivery.

The ideal candidate has an accredited nursing degree (ASN/BSN), an active RN license, CPR certification, and at least two years of clinical RN experience, preferably in hospice or home health, with strong computer and

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles

Responsibilities

• Administer skilled nursing care in clients' homes or residences.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care across settings as needed.
