Job summary

Moments Hospice in Bloomington, MN seeks an on-call RN to organize and direct hospice care during nights and weekends, with a 7 days on / 7 days off schedule. The role includes after-hours calls, home visits, and coordination with the interdisciplinary team.

You will provide nursing care under the physician's plan, maintain EMR documentation, and counsel families. A company car is provided for business and personal use at low cost, along with comprehensive benefits.

Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs for after-hours calls.
• Responds to calls from Triage during assigned shifts and makes visits to patients with changing conditions.
• Provides professional nursing care by utilizing all elements of nursing process.
• Develops and initiates plan of care and revises as patient needs change.
• Administers medications and treatments as prescribed by the physician.
• Coordinates with physician and community care teams to meet patient needs.
• Documents patient status and care provided; communicates updates to the team.
