Moments Hospice is seeking an experienced registered nurse to provide high-quality hospice care in home settings, covering after-hours and weekend shifts.

This full-time role involves visiting patients, coordinating with families and the interdisciplinary team, and maintaining thorough documentation.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Perform initial and ongoing patient assessments to determine hospice needs.
• Respond to after-hours calls and manage home visits.
• Provide nursing care per the plan of care and physician orders.
• Develop and revise care plans with patient and family involvement.
• Deliver patient and family education on self-care and medications.
• Collaborate with Interdisciplinary Group to meet emotional needs.
