Job summary

Moments Hospice is seeking an on-call Registered Nurse to cover nights and weekends, coordinating hospice care for patients and their families in homes and communities. The RN will perform comprehensive assessments, develop and revise care plans, administer medications, and educate families while working with an interdisciplinary team.

The role is a Full-Time, on-call position with a 7 days on/7 days off schedule, including evenings and weekends.

Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met.

Responsibilities

• Performs initial and ongoing assessments of patient and family for after-hours hospice needs.
• Responds to calls from triage during shifts; makes patient visits as conditions change.
• Provides nursing care using the nursing process.
• Develops and revises the plan of care with patient and family involvement.
• Administers medications and treatments as prescribed.
• Educates patient and family on self-care and health maintenance.
• Coordinates with physician and interdisciplinary team for ongoing care.
