Job summary

Transitions Hospice is seeking an adaptable RN to join our Extended Care Team as an ECT Nurse covering Fort Wayne and surrounding areas. This PRN role offers flexibility to respond quickly to patient needs, with every visit making a real difference in comfort and outcomes.

You'll perform comprehensive nursing assessments, administer medications per physician orders, collaborate with an interdisciplinary team, coordinate with families and partners, and document clearly to support smooth

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• Experience in hospice, home health, or related setting preferred.
• Flexibility and confidence working across varied patient assignments.
• Strong assessment skills and sound clinical judgement under pressure.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct thorough patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and necessary services are arranged and available for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.
