Job summary

Transitions Hospice is seeking an experienced RN to join our Extended Care Team as an ECT Nurse in Pekin, IL. You will respond to needs across Washington, East Peoria, Pekin, and surrounding areas, delivering skilled nursing care in diverse settings on a PRN as needed basis.

You must hold an active RN license in Illinois, graduate from an accredited school of nursing, and be flexible, with strong assessment, documentation, and communication skills.

Qualifications

• Active RN license in the state of employment.
• Clinical skills in hospice/home health or related setting preferred.
• Strong documentation and communication abilities.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Assess patients and families, contributing to individualized care plans.
• Complete all visit types to meet patient/family needs.
• Administer medications and treatments per orders.
• Record accurate clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners for seamless transitions.
• Ensure equipment and services are arranged for patients.
• Continue education to grow clinical skills.
