Transitions Hospice - Indiana is seeking an experienced RN to join our Extended Care Team (ECT) as a PRN nurse serving Fort Wayne and surrounding areas. You’ll provide flexible, high-quality nursing care across varied patient assignments, stepping in where the regular team needs reinforcement.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Validity to travel locally and operate in varied environments.
• Strong assessment and clinical judgment under pressure.
• Excellent documentation and communication skills.
• Valid driver's license.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding accordingly.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support seamless care transitions.
• Ensuring equipment and necessary services are arranged and available for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.
