Qualifications

• Graduate of an accredited nursing program with current state licensure.
• Minimum one year of nursing experience; three+ years preferred.
• Hospice experience preferred.
• Knowledge of the hospice care philosophy.
• Commitment to clinical and documentation excellence.
• Must have a valid driver's license with insured vehicle.

Responsibilities

• Provide 1:1 end-of-life care during after-hours on-call schedule.
• Assess and manage urgent hospice symptoms with compassion.
• Administer or adjust medications per hospice protocol.
• Communicate changes with the physician and obtain orders.
• Support patient and family, explain current condition and next steps.
• Pronounce death when needed and notify the hospice team.
• Complete timely documentation and communication.
