Job summary

Three Oaks Hospice is looking for a compassionate On-Call Registered Nurse to join our hospice care team serving patients in the Woodstock area and nearby territories. The role involves delivering skilled nursing care in patients' homes, coordinating with the interdisciplinary team, and supporting families during difficult times.

Requirements include an Associate or Bachelor’s degree in Nursing, an active RN license, CPR certification, and at least two years of RN experience; strong computer

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft Office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence; coordinate care with the interdisciplinary team, patient/family and referring agency.
• Coordinate care with the interdisciplinary team and family to ensure seamless patient care.
• Provide compassionate support to patients and families while managing care coordination and documentation.
