Job summary

UnitedHealth Group (TotalCare) is seeking an On Call Registered Nurse to deliver and direct in-home nursing care as prescribed by physicians and compliant with laws and agency policies. You will coordinate the total plan of care with the care team and ensure continuity by communicating with physicians, families, and community resources.

The role offers competitive compensation, with additional pay for on-call rotations and completing patient visits while on call.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license with vehicle insurance or access to reliable transportation.
• CPR certification preferred or obtainable within 90 days of employment.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency/duration meets patient needs and revise with physician approval.
