UnitedHealth Group's Home Health RN role delivers and directs nursing care to patients in their homes, coordinating the total plan of care with physicians and the health care team to maintain continuity of patient care.

You will conduct comprehensive assessments, supervise home health staff, document visits, and participate in on-call rotations. A valid RN license in the state and reliable transportation are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR Certification or ability to complete within 90 days of hire.
• Preferred: 1+ years of Home Health experience.
• Preferred: Ability to work independently.
• Preferred: Strong communication, writing, and organizational skills.

Responsabilités

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services within the plan of care and infection control standards; completes comprehensive assessments (OASIS).
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
• Makes initial/comprehensive nursing evaluations to determine eligibility and medical necessity; communicates with staff, physicians, and family.
