Job summary

Elara Caring is seeking a Registered Nurse for After-hours on-call visits in a home health setting. The role focuses on 1:1 patient care across greater Lansing, St. Johns, and nearby areas with a 7 days on/7 days off schedule.

You will cover 2:00pm–8:00am on-call windows, conduct initial and ongoing assessments, and collaborate with the clinical team to ensure high-quality, safe patient outcomes in the home setting.

Qualifications

• Graduate of an accredited nursing program with active RN license.
• 1+ year of nursing experience in a clinical setting.
• Ability to travel approximately 50% and cover shifts from 2:00pm–8:00am when on-call.
• Strong verbal and written communication skills.

Responsibilities

• Assess caseload of home health patients and document needs.
• Perform initial evaluation and re-evaluate nursing needs at visits.
• Communicate findings to supervisor, physician, and care team.
• Counsel and implement the plan for patient safety and care.
• Follow physician orders and standards to deliver quality care.
