Qualifications

Responsibilities

• Completes initial patient and family assessments including OASIS assessments as appropriate.
• Regularly re-evaluates patient nursing needs and revises the plan of care.
• Administers medications and treatments as prescribed by the physician.
• Coordinates care and communicates with physicians, patients, and interdisciplinary team.
• Supervises home health aides and ensures proper documentation.
