Job summary

Responsibilities

• Completes initial assessment of patient and family to determine home care needs and conducts OASIS assessments as appropriate.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises it as patient status changes.
• Coordinates services and makes referrals to other services as needed.
• Administers medications and treatments as prescribed by the physician.
• Acts as Case Manager for assigned caseload and collaborates with physician and interdisciplinary team.
