Job summary

Cantex Continuing Care Network is seeking an experienced on-call Hospice RN to provide skilled nursing care to patients in their home within the Sherman/Denison area. You will coordinate with the interdisciplinary team, physicians, patients, and families to develop and manage hospice plans of care and ensure regulatory compliance.

Qualifications

• Licensed registered nurse (RN) in state requirements.
• Hospice experience preferred.
• Home Care Home Base (HCHB) experience preferred.
• Strong clinical assessment, communication and documentation skills.
• Experience working with an interdisciplinary healthcare team.
• Knowledge of hospice regulations and care planning.

Responsibilities

• Perform initial and ongoing comprehensive nursing assessments for hospice patients.
• Develop and update individualized hospice plans of care for quality and continuity.
• Coordinate patient care with IDT, physicians, patients, and families.
• Educate patients and families on disease, medications, symptoms, and end-of-life care.
