Job summary

Bay Area Community Services is seeking On-Call Clinical Care Coordinators in Oakland, providing high-quality therapeutic interventions to support wellness and recovery. The role includes developing services across individual, family, and group modalities within a 24-hour Crisis Stabilization Unit.

On-Call shifts are swing 4pm–midnight and night 12am–8am, with a minimum of two shifts per month; additional shifts may be available at crisis residential locations in Vallejo and Hayward.

Qualifications

• MSW, MFT, MPCC required.
• ASW/LCSW or AMFT/LMFT with current BBS registration; and 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• Healthcare/vulnerable client employer, verified COVID vaccination required.

Responsibilities

• Develop and deliver a full complement of services including individual, family, group counseling, and case management for assigned caseload.
• Utilize Evidence-Based Practice to assure high-quality results and impact.
• Assess risk status and intervene appropriately.
• Create service plans aimed at client recovery and goal achievement.
• Engage and report disengagement to the team; coordinate disenrollment per protocol.
• Coordinate care planning and therapeutic case management with a flexible approach.
• Document data and maintain records per policies with required face-to-face service time.
• Participate in supervision, meetings, travel, crisis coverage, and training.
