Job summary

Qualifications

• You have a high school diploma or GED.
• You have one year's experience working in health care, home health, or mental health service settings with an adult population who has experience a severe and persistent mental illness.
• Valid driver’s license with a 5-year clean driving record.

Responsibilities

• Provide assertive outreach to engage individuals in the community and address barriers to housing and healthcare access/outcomes.
• Provide case management to connect clients to resources and build skills to address functional impairments and quality of life.
• Assist clients in meeting project goals including housing, employment and other social welfare needs.
