Responsibilities

• Assess client needs and develop tailored care plans.
• Coordinate with social workers, doctors, and psychologists to plan health services.
• Research, refer, and maintain relationships with community resources and government agencies.
• Maintain precise clinical documentation and scheduling within EHR systems.
