SYNERGY HomeCare of Madison in Monona, Wisconsin is seeking an On-Call Caregiver to provide compassionate, in-home support to clients with a focus on individualized care plans and safety. You will perform activities of daily living assistance, document client progress, and coordinate with a team of caregivers and healthcare professionals to ensure seamless care.

Qualifications

• High school diploma or equivalent
• Prior healthcare experience, preferably caregiving
• Ability to lift up to 25 pounds and perform physical demands
• Valid driver's license and reliable transportation
