SYNERGY HomeCare of Madison in Monona, Wisconsin is seeking an On-Call Caregiver to provide compassionate, in-home support to clients with a focus on individualized care plans and safety. This role emphasizes reliable, empathetic service in clients' own homes and contributes to meaningful outcomes for families.

You will perform activities of daily living assistance, document client progress, and coordinate with a team of caregivers and healthcare professionals to ensure seamless care.

Qualifications

• High school diploma or equivalent
• Prior healthcare experience, preferably caregiving
• Ability to lift up to 25 pounds and perform physical demands
• Valid driver's license and reliable transportation

Responsibilities

• Provide one-on-one care and companionship to clients in their homes, focusing on their unique needs and abilities
• Assist with daily living activities, including personal care, meal preparation, and light housekeeping
• Encourage and support clients in their daily routines, promoting independence and autonomy
• Maintain accurate and detailed records of client care, including progress notes and treatment plans
• Collaborate with other caregivers and healthcare professionals to ensure seamless care coordination
• Demonstrate a commitment to excellence, prioritizing client satisfaction and well-being above all else
