Job summary

Redwood Community Services, Inc. is seeking an On-Call Care Manager to provide rehabilitative and crisis intervention services in residential and community settings. This flexible position offers on-call coverage and a chance to support at-risk individuals with personalized case management.

The role requires driving, a CA driver's license, background checks, CPR/First Aid, and the ability to work across agency programs. Pay reflects experience and differential pay for bilingual skills.

Qualifications

• Care Manager I: < 2 years FTE experience in mental health direct services.
• Care Manager II: ≥2 and <4 years FTE experience in mental health direct services.
• Care Manager III: ≥4 years FTE experience in mental health direct services; up to 2 years may be substituted by education in a mental health-related field; minimum 2 years actual work experience; education requirement: minimum 60 semester/90 quarter units in mental health-related subjects.

Responsibilities

• Provide client-centered rehabilitative support by assessing client needs, promoting positive behaviors and social interaction, and helping individuals develop essential life skills.
• Plan and lead activities and outings, support meal planning and grocery shopping, assist with medication as needed, complete required documentation, ensure services comply with State and County Mental Health guidelines.
