Job summary

Qualifications

• Care Manager I: <2 years FTE in mental health direct services.
• Care Manager II: ≥2 and <4 years FTE in mental health direct services.
• Care Manager III: ≥4 years FTE in mental health direct services; up to 2 years of the four years can be substituted by mental health-related education.
• Education requirement: minimum 2 years in mental health-related subject areas (e.g., psychology, social work, rehab).

Responsibilities

• Provide client-centered rehabilitative support by assessing client needs and promoting positive behaviors and social interaction.
• Help individuals develop essential life skills; plan and lead activities and outings.
• Support meal planning and grocery shopping; assist with medications as needed; complete required documentation.

Education

Two years of education in a mental health-related field (minimum 60 semester credits or 90 quarter units)
